Student Served Survey - Students who meet with Advising & Counseling Services can complete the Student Served Survey and give feedback on their visits. Students may complete this survey before leaving the office or by linking to it online. This information is used to collect data on student experiences and to continue to improve our services. The last day to complete this survey for the Spring 2009 and Summer 2009 Sessions will be Tuesday, September 1, 2009 at 5:00 p.m.

Career and Transfer Services Pre-Counseling Form - Students who meet with a Career & Transfer Counselor need to complete the attached form prior to their first meeting.  Students may complete the form immediately prior to the counseling session or by linking to it online. This form will give the Counselor an overall picture of the student’s background which will contribute to the counseling session.

Tools for Journey Survey - Students who attend the “Tools for the Journey” events on Tuesday, September 29, 2009 and/or Thursday, October 1, 2009 must complete this survey. Those students who leave the “Tools for the Journey” event before completing this survey may complete the survey online.

College Information Day Survey - Students who attend the College Information Day event on March 4, 2010 from 11:30 a.m. – 1:30 p.m. as part of the Spring 2010 Student Success Week must complete the College Information Day Survey before leaving the event. Students who must leave the College Information Day before completing the survey can complete it online. Responses for this survey must be completed by March 4, 2010 at 5:00 p.m.

Career Fair Event Survey - Students who attend the Career Fair event on Thursday, April 16, 2009 from 4:30 to 6:30 p.m. should complete this Career Fair Survey before leaving the event. Students who must leave the Career Fair before completing this survey the day of the event may also complete it online. Responses for this survey must be completed by Friday, April 17, 2009 before 5:00 p.m.
